- Widescreen and bigger resolutions than original 800x600 are supported, enhanced HD text and 60+ FPS.
- Loot filters can hide undesirable items and mark items with colors as well as display drops on the mini map or chat.
- Game name/password of the last game you joined/created will be remembered for easy re-joining.
- DPS meter has been added, type .dps to enable/disable it.
- Quick Cast for skills and buff duration timers have been added. New item transfer keybinds and many other settings like showing/hiding item level. You can bind every skill to left click, including Paladin auras (can't use two auras).
- You can stack runes/gems/misc items and cubing runes now requires keys (sold by Akara) instead of gems. Default keybinds: pick up the item to unstack then press Ctrl+left click and on hover Shift+Ctrl+left click to add/remove plus.
- Killing the Cow King will no longer prevent you from opening the Secret Cow Level.
- Perfect gems drop on Hell and Flawless gems start dropping in A4 Normal.

- Ethereal items gain 25% damage/defense bonus instead of 50%. They also have a full durability instead of 50% and most of the items had their durability doubled. You can repair ethereal (even broken) items by cubing them with Zod Rune and Perfect Skull.
- Throwing weapons and arrows/bolts have unlimited quantity so every ranged weapon is better if it's ethereal because it can't break.
- Runewords made in staves will cast spells on casting instead of striking. Staves always have 10/30/50% FCR mod (normal/exceptional/elite).
- Sanctuary aura reduces enemy Magic Resistance.
- Polearm runewords can also be made in spears.
- Movement speed debuff automod was removed from medium and heavy armors, allowing you to wear any armor you like without penalty.
- Necromancers can curse monsters with multiple curses at once. Curse Resistance reduces effects of curses, it replaced Reduced Curse Duration corruptions and is now capped at 75%. Monster curses will last 20 seconds.
- Summons and traps gain bonuses from plus/minus elemental Skill Damage % like Griffon.
- Rare items will always roll minimum of 4 affixes at item level 45+, 5 at 65+ and 6 at 85+. Rare jewels will always roll 4 affixes.
- Crafting recipes no longer increase level required.
- Open Wounds (OW) stacks up to 3 times, lasts for 5 seconds and works with splash damage.
- Crushing Blow (CB) now caps at 65% versus bosses and works with splash damage.
- Deadly Strike (DS) and Critical Strike (CS) are capped at 75% instead of 100% and DS deals 1.5 times more damage instead of double.
- Resistance cap has been lowered from 95% to 90%.
- Rejuvenation potions can't heal more than 2000 health.

- Torch and Anni give 30-60 Vitality instead of 10-20 Str+Dex+Vit, also Torch now gives only +2 to class skills instead of +3.
- Spirit gives +1 skills instead of +2, Lidless can now give +2 skills.
- Arach rolls 15-20% FCR but corruption can make it 30% FCR.
- Life Tap has been removed from most of the items like Dracul or Exile but Last Wish (LW) still grants it. To compensate, items give much more Replenish Life.
- Decrepify curse has been replaced with Amplify Damage on most of the items. It's a very valuable curse for physical damage dealers and every melee should have it on switch or on merc.
- Soj and BK are not the only +1 skills rings anymore as Wisp also gives this bonus, moreover every ring can get additional 10% FCR from corruption and rare rings can get 20%.
- Templar's Might now gives Might aura when equipped, making it a great choice for merc armor.
- Paladin shields can get only up to 20-30% all resistances automod (down from max 45%).
- Necro exceptional and elite shields can now roll up to 3 sockets.
- Aegis and Ward shields have better block chance and defense.
- Clubs roll Druid skill mods.
- Arrows and Bolts may be rare or magic and A1 rogues can equip them.
- Large Charms can spawn new prefix: up to 4% to Magic, Poison, Cold, Fire or Lightning Damage.
- Rare and magic gloves, shields, belts and armors can roll 10-20% FCR and 10-20% Damage Reduction.
